Why we collect this information
FlairsTech collects dependent information solely to enrol and administer your family members under the company medical insurance policy, and to calculate any employee contribution deducted through payroll.
What we collect
Relationship, full legal name, date of birth, national ID (where required), nationality (where the insurer requires it), and the supporting documents listed on the request form. We do not collect information that is not needed for insurance administration.
Who can see it
Authorized FlairsTech HR / Insurance staff and the contracted insurance provider. Every access to identity documents and full national IDs is logged.
How it is protected
National IDs and documents are encrypted at rest. Documents are never sent by email and are only viewable inside this application after a permission check.
Children's data
Information about children is treated as sensitive personal data. By submitting, you confirm you are the parent or legal guardian authorized to provide it.
Retention and your rights
Records are retained for the period approved by FlairsTech HR / Legal for insurance administration. You may request access to or correction of your data by contacting the HR / Insurance team.
Declaration you confirm on submission (v2026-10-v1)
I confirm that the information and documents I provided are accurate and belong to me or to a dependent for whom I am authorized to submit this insurance request. I understand that the information will be processed by FlairsTech and authorized insurance-processing parties for administration of the medical insurance benefit.
This notice is pending final approval by FlairsTech HR / Legal / DPO before production use.
